Please first use this link below when requesting courtesy waivers and must be requested within 24 hours with no exceptions (unless the link does not work, then go to AVA):

Please follow the steps below by completing everything that is asked for in order for the waiver to be processed correctly:
1-Once you click on the link above it will take you to the login page: enter your username and password like logging into the Arise portal.
2-Click on the drop down arrow and select the reason for the waiver request
3-Choose the client program name
4-Click on the calendar and select the start date and start time
5-Answer if the issue is ongoing
6-Click on the calendar and select the end date and end time of the last interval, for example: if the interval starts at 9:00am, then you would indicate the end time as 9:30am.
7-Click to the top right corner "Submit"
8-After clicking "Submit" a new page opens and should see message "Thank you, your request has been submitted"
Last step 9 very important to get the courtesy INC#, NOT the REQ#.
9-Click under "Description" the link "Complete this form to request a Courtesy Waiver" and a new page opens and please scroll all the way to the bottom of the page and write down this important courtesy INC# assigned to our request and please open a DESK ticket using the "INC and Courtesy" template for me to follow up on the waiver by providing the INC# in the DESK ticket subject line, date of INC# requested and for what date it applies to and then the time frame affected.
